Hey! I have an entry in the LP deviantART contest, and in the rules say there is an entry form to fill out, I found the waiver but not the entry form..can you tell me where to find it? And do you only fill out and send the waiver if you win? Thanks for your help!

In the rules it states: All entrants will also be required to complete an entry form that must include entrants Name, Age, Street Address, City, State, Zip Code and Telephone Number, Mobile Number and Email Address (Optional).

Where is this entry form and how/who do we send it to??? I have an entry in the deviantart contest and I want to make sure I have complied with all of the rules.

They told me everything we need to know is on the deviantart site... after looking it over again I am assuming the entry form was the one to join deviantart..... it fulfills all of the info they were asking for. I could be wrong, but it is the only option left.
